Wirkungsvergleich vonl-dopa und der kombinationl-dopa + decarboxylasehemmer beim Parkinson-syndrom

Comparison of effects of levodopa and the combination of levodopa and a decarboxylase inhibitor in Parkinson's disease

Summary

In a group of 70 parkinsonian patients 40 were treated with levodopa and 30 with the combination of levodopa and an inhibitor of peripheral decarboxylase (Ro 4-4602) in the ratio 4∶1. At regular intervals these patients were subjected clinicaly, neuropsychologic, and electromyographic examinations. After 3 monts of treatment the two modes of administration showed approximately the same effect on the clinical symptoms. There is a significantly more rapid improvement under treatment with the combination. The decarboxylase inhibitor reduces the equivalent dose of levodopa to 1/5. Treatment with levodopa alone is often limited by gastrointestinal intolerance. Under the combination gastrointestinal intolerance and hypotension are very slight, whereas abnormal involuntary movements are more frequent and severe.

Zusammenfassung

Von 70 Parkinson-Kranken wurden 40 mitl-Dopa und 30 mit der Kombinationl-Dopa/Decarboxylasehemmer (Ro 4-4602) im Verhältnis 4∶1 behandelt und zu festgelegten Terminen klinisch, testpsychologisch und elektromyographisch untersucht. Nach 3 monatiger Therapie haben beide Verfahren einen etwa gleich positiven Effekt auf die klinische Symptomatik. Die Besserung tritt bei der Kombinationsbehandlung jedoch signifikant schneller ein. Die gleichzeitige Gabe eines Decarboxylasehemmers reduziert die äquivalentel-Dopa-Dosis auf 1/5. Die reinel-Dopa-Therapie wird häufig durch gastrointestinale Intoleranzerscheinungen limitiert. Diese Nebenwirkungen spielen bei der Kombinationsbehandlung ebenso wie hypotone Kreislaufregulationsstörungen eine nur geringe Rolle, während extrapyramidale Hyperkinesen häufiger und in stärkerer Ausprägung auftreten.
