Abstract Summary Cooperation in the stroke service: a unique couple

A new form of cooperation in the stroke service is one between a rehabilitation centre and a nursing home, whereby the rehabilitation department of the nursing home is localized within the building of the rehabilitation centre. In this paper a study into the feasibility of structural cooperation between to two care providers is reported. The possibilities for structural coordination of care are mainly found on the level of the individual patient, since the financial structures of both centres are different. The coordination of care for stroke patients can be optimized by this unique coordination of care. The ultimate goal is to provide the proper care, at the proper moment and the proper place.

Samenvatting

Een nieuwe variant in de stroke service is de samenwerking tussen een revalidatiecentrum en een verpleeghuis in de keten, waarbij de reactiveringsafdeling van het verpleeghuis haar ‘intrek’ heeft genomen in het revalidatiecentrum. In dit artikel wordt gerapporteerd over een haalbaarheidstudie naar een mogelijke structurele samenwerking in de toekomst, waarbij de volgende vraag centraal stond: kan de ketenzorg en met name de doorstroom van patiënten, geoptimaliseerd worden door de samenwerking tussen beide instellingen te intensiveren? De mogelijkheden voor samenwerking worden vooral op het niveau van de individuele patiënt gezien en op het niveau van onderlinge afspraken voor afstemming van de zorg. Deze unieke samenwerking kan de ketenzorg ten goede komen doordat de CVA patiënt de juiste zorg, op de juiste plaats en op het juiste moment kan ontvangen.
