Gore Bio-A Fistelplug zur Behandlung hoher Analfisteln

Erste Ergebnisse

Zusammenfassung

Hintergrund

Die Behandlung hoher Analfisteln ist mit einem relevanten Risiko für eine Stuhlinkontinenz behaftet. Neben den konventionellen Verfahren des plastischen Fistelverschlusses stellt die Okklusion mittels Fistelplug ein neues sphinkterschonendes Verfahren dar. In dieser Studie werden erste Ergebnisse vorgestellt.

Patienten und Methode

Zwischen November 2010 und Mai 2011 wurde bei 12 Patienten (9 m, 3 w, Alter: 52±13 Jahre) ein Fistelverschluss mit dem Gore Bio-A Fistelplug durchgeführt. Es handelte sich in allen Fällen um hohe transsphinktäre Analfisteln. Eine chronisch-entzündliche Darmerkrankung war bei allen Patienten ausgeschlossen worden. Sieben Patienten waren präoperativ vollkommen kontinent, die übrigen 5 wiesen eine Inkontinenz für Luft auf (Wexner-Score: 4–9).

Ergebnisse

Im postoperativen Verlauf kam es bei einem Patienten zu einem Sekretverhalt der äußeren Wunde, der operativ behandelt werden musste. Vier Patienten klagten bei reizlosen Wundverhältnissen über vermehrte Schmerzen. Bei der Nachuntersuchung nach 6 Monaten war bei 6 Patienten die Fistel sicher verheilt. Die übrigen Fisteln waren als persistent anzusehen und wurden einem erneuten Eingriff zugeführt. Die Heilungsrate betrug somit 50%. Bei keinem der Patienten kam es zu einer Verschlechterung der präoperativen Kontinenzleistung.

Schlussfolgerung

Durch die Fistelokklusion mittels Plug konnte bei 50% der Patienten eine definitive Heilung der Fistel erzielt werden. Es wurde keine negative Beeinflussung der Stuhlkontinenz beobachtet, so dass aus unserer Sicht die Implantation eines Fistelplugs als primäre Therapieoption bei hohen Fisteln diskutiert werden muss.
