Zusammenfassung

Die Mitralklappeninsuffizienz (MI) mit resultierender Herzinsuffizienz ist eine der häufigsten Herzklappenerkrankungen. Verschiedene Techniken der katheterbasierten Therapie einer MI stehen bereits heute bei Hochrisikopatienten zur Verfügung. Die meiste Erfahrung liegt aktuell mit dem MitraClip®-System vor. Die technologischen Entwicklungen auf dem Gebiet der Kathetertherapie der MI laufen rasant weiter. Die Verfahren eignen sich für Patienten mit einer sowohl primären als auch sekundären MI. Goldstandard in der chirurgischen Therapie ist jedoch insbesondere bei sekundärer MI die Annuloplastie. Bei der katheterbasierten Therapie der sekundären MI werden die indirekte und die direkte Annuloplastie unterschieden. Aktuell entspricht der chirurgischen Ringimplantation am ehesten die direkte Annuloplastie. Die katheterbasierte Implantation einer Mitralklappe ist heutzutage bereits möglich, jedoch existieren nur wenige Berichte und begrenzte Erfahrungen. Die Entwicklung im Mitralklappenersatz verläuft gegenüber der katheterbasierten Aortenklappenimplantation deutlich langsamer. Ursächlich hierfür ist der komplexere anatomische Aufbau der Mitralklappe. Gute Erfahrungen gibt es bereits mit der Implantation einer katheterbasierten Aortenklappenbioprothese bei Patienten mit degenerierten Mitralklappenbioprothesen oder insuffizient gewordenen chirurgischen Annuloplastieringen („valve-in-valve“/„valve-in-ring“). Es ist zu erwarten, dass in Zukunft weitere katheterbasierte Techniken zur Therapie einer MI bei Hochrisikopatienten zur Verfügung stehen werden.
