Infektionsprävention und -Surveillance bei Kunstherzpatienten

Zusammenfassung

Kunstherzimplantationen stellen eine neue etablierte Therapieoption der terminalen Herzinsuffizienz dar. Jedoch ist die Implantation von Kunstherzen („ventricular assist device“, VAD) mit einer der höchsten postoperativen Infektionsraten verbunden. Die Zusammenstellung publizierter Infektionszahlen zeigte Infektionsinzidenzen zwischen 20 und 188%. Somit kommt infektionspräventiven Maßnahmen eine entscheidende Bedeutung zu. Da zurzeit kaum Studien existieren, in denen infektionspräventive Maßnahmen an VAD-Patienten untersucht werden, wird in der vorliegenden Arbeit versucht, in anderen Patientenkollektiven evaluierte Präventionsmaßnahmen auf VAD-Patienten zu übertragen. Es werden die folgenden Präventionsmaßnahmen diskutiert: Indikationsstellung, perioperative Antibiotikaprophylaxe, postoperative Wundpflege an der „Drive-line-Austrittsstelle“, Endokarditisprophylaxe sowie Schulung des Patienten, des pflegenden Personals und der Angehörigen. Für die „surveillance“ von Post-VAD-Implantationsinfektionen wird ein Erfassungssystem vorgeschlagen, das die unterschiedlich langen VAD-Trageintervalle abbildet und somit einen fairen Interzentrumsvergleich gestattet.
