Fast-Track-Rehabilitation am Beispiel der Kolonchirurgie

Zusammenfassung

Fast-Track-Rehabilitation bezeichnet ein interdisziplinäres multimodales Vorgehen zur Verbesserung und Beschleunigung der Rekonvaleszenz und zur Vermeidung perioperativer Komplikationen. Das Konzept zielt auf eine reduzierte Morbidität und schnellere Entlassung der Patienten aus dem Krankenhaus ab. Es beinhaltet die präoperative Information des Patienten, eine atraumatische chirurgische Technik, die Verminderung von Stress, Schmerzausschaltung – meist durch regionalanästhesiologische Techniken (vorwiegend in Form der thorakalen Epiduralanästhesie), optimiertes Flüssigkeits- und Temperaturmanagement, frühe enterale Ernährung, Prophylaxe von gastrointestinaler Atonie sowie postoperativer Übelkeit und Erbrechen, schnelle postoperative Mobilisation und frühere Entlassung aus dem Krankenhaus. Fast-Track-Protokolle gibt es mittlerweile für viele chirurgische Eingriffe. Am besten etabliert sind sie für die Kolonchirurgie.
