Tako-Tsubo-Kardiomyopathie

Zusammenfassung

Die Tako-Tsubo-Kardiomyopathie (TK) ist eine akut auftretende kardiale Erkrankung. Die damit verbundene myokardiale Funktionseinschränkung kann klinisch zunächst kaum von einem akuten Myokardinfarkt unterschieden werden. Die TK tritt hauptsächlich bei postmenopausalen Frauen auf und hat meistens eine gute Prognose. Die Pathogenese ist bisher nicht eindeutig geklärt, aber eine durch erhöhte Katecholaminspiegel induzierte myokardiale Schädigung ist die bisher favorisierte Hypothese. Zahlreiche Erkrankungen und emotionaler Stress, aber auch chirurgische Eingriffe und Narkose wurden als potenzielle Auslöser einer TK beschrieben. Daher ist die Kenntnis dieses Krankheitsbilds auch für den Anästhesisten von besonderer Bedeutung. Über die beste Therapie der TK ist bisher wenig bekannt. Sie unterscheidet sich aber möglicherweise von den Therapiealgorithmen der normalen Herzinsuffizienz. Kenntnisse über die TK als Differenzialdiagnose zum akuten Myokardinfarkt sind notwendig, um Fehlbehandlungen zu vermeiden.
