Basisalgorithmus für „Point-of-Care“ basierte Hämotherapie

Perioperative Versorgung koagulopathischer Patienten

Zusammenfassung

Bei der perioperativen Versorgung koagulopathischer Patienten ermöglicht die Verwendung bettseitig verfügbarer Verfahren zur Gerinnungsdiagnostik [“Point-of-Care“(POC)-Verfahren] eine im Vergleich mit der konventionellen Routinegerinnungsdiagnostik [“activated partial thromboplastin time“ (aPTT), Quick-Wert, International Normalized Ratio (INR), Fibrinogenkonzentration, Thrombozytenzahl] schnellere und umfassendere Analyse von Gerinnungsstörungen. In der vorliegenden Übersicht wird ein Hämotherapie-Algorithmus präsentiert, der auf viskoelastischen und aggregometrischen POC-Verfahren basiert. Der Hämotherapie-Algorithmus besteht aus einem allgemeinen und einem speziellen Teil. Der allgemeine Teil hält Felder für die Patientenidentifikation vor und gibt allgemeine Hinweise zum Gerinnungsmanagement sowie zur Modifikation des Algorithmus bei speziellen Patientengruppen und präsentiert außerdem Therapievorschläge für die Notfallantagonisierung antikoagulativer Dauermedikation. Im speziellen Teil werden neben den physiologischen Rahmenbedingungen für Hämostase Messergebnisse von Analysen der Gerinnungsaktivierung, Gerinnselstärke, -stabilität und Thrombozytenfunktion abgefragt. Für jeden abgefragten Parameter werden Referenzwerte angegeben und Therapieoptionen vorgeschlagen. Bei Persistenz der Koagulopathie trotz algorithmuskonformer Therapie kann der Algorithmus erneut durchlaufen werden; es werden ferner Therapievorschläge zur Ultima-Ratio-Therapie präsentiert.
