Deutschlandweite Umfrage zur Thoraxanästhesie

Zusammenfassung

Hintergrund

Ziel der Arbeit ist die erstmalige deutschlandweite Erhebung thoraxanästhesiologischer Behandlungsstrukturen und -abläufe sowie die Quantifizierung möglicher Unterschiede zwischen den verschiedenen Krankenhausversorgungsstufen.

Material und Methodik

Alle (n = 777) bei der DGAI registrierten anästhesiologischen Chefärzte wurden eingeladen, einen 28 Fragen umfassenden, vierteiligen Onlineumfragebogen zu beantworten.

Ergebnisse

Die Antwortrate lag bei 31,5 % (n = 245). Thoraxchirurgische Eingriffe werden vorwiegend durch anästhesiologische Fach- und Oberärzte (59,9 %) betreut. Zur Einlungenventilation wird überwiegend der Doppellumentubus (91,4 %) genutzt (Bronchusblocker: 2,7 %; keine Angabe: 5,9 %). Spezielle Algorithmen für den schwierigen thoraxanästhesiologischen Atemweg sind eine Rarität (4,8 %). Bronchusblocker sind nur in 64,7 % der thoraxanästhesiologischen Kliniken vorrätig. Während Kliniken niedrigerer Versorgungsstufe die Patienten zumeist direkt auf eine Intensivstation zur Überwachung verlegen (92,3 %), versorgen Krankenhäuser der Maximalversorgung einen größeren Anteil der Patienten im Aufwachraum (12,5 %) oder auf Überwachungsstationen (14,6 %). Zur peri- und postoperativen Schmerztherapie wird die Periduralanästhesie (85,6 %) der Paravertebralblockade (Single-Shot: 8,0 %; Katheter: 8,6 %) vorgezogen (Mehrfachantworten möglich). Zudem wurde deutlich, dass ein großer Bedarf an konkreten Empfehlungen zum Vorgehen beim schwierigen thoraxanästhesiologischen Atemweg und an allgemeinen Aus- und Weiterbildungsmöglichkeiten besteht.

Diskussion

Es zeigten sich wesentliche Unterschiede in der anästhesiologischen Betreuung thoraxchirurgischer Patienten. Diese bezogen sich insbesondere auf die postoperative Überwachung, die Verfügbarkeit von Bronchusblockern sowie den Einsatz regionalanästhesiologischer Techniken. Eine Empfehlung für den materialintensiven und hochtechnisierten thoraxanästhesiologischen Arbeitsplatz könnte die Strukturqualität und folglich den Behandlungserfolg für den Patienten optimieren. Unabhängig von der jeweiligen Versorgungsstufe der einzelnen Klinik könnten einheitliche Vorgaben helfen, nationale Qualitätsstandards in der Thoraxanästhesie zu etablieren.
