Kardiovaskuläre Hochrisikokonstellationen in der primärärztlichen Versorgung

DETECT-Studie 2003*

Zusammenfassung

DETECT ist ein epidemiologisches Querschnitts- (Stichtags-) und Längsschnittsstudienprogramm im primärärztlichen Versorgungssektor. Auf der Grundlage einer bundesweiten Zufallsstichprobe von 3795 Arztpraxen (Teilnehmerrate 60,2%) wurde im September 2003 der Gesundheitszustand von n=55.518 (Response Rate 93,5%) Patienten standardisiert (Fragebögen, Arztinterview, Messungen) untersucht. Ziel war die Ermittlung der Stichtagsprävalenz und Komorbidität sowie der Versorgungsgüte von Patienten mit Diabetes mellitus, arterieller Hypertonie, Hyperlipidämie und koronarer Herzkrankheit sowie die Beurteilung behaviouraler, klinischer, labortechnischer und psychologischer Risikofaktoren und -scores. Eine randomisierte Teilstichprobe (n=7519) dieser Patienten wurde darüber hinaus standardisiert umfassend mittels Laboranalysen charakterisiert, über ein Jahr hinweg klinisch verfolgt und nach 12 Monaten nachuntersucht. Erste Ergebnisse bestätigen die hohe Prävalenz von Hypertonie (36,3%), Hyperlipidämie (29,1%), Diabetes mellitus (14,6%) und der koronaren Herzkrankheiten (12,4%) sowie ihre ausgeprägten Assoziationen im primärärztlichen Sektor. DETECT erlaubt es erstmals, für den primärärztlichen Bereich die Größenordnung kardiovaskulärer Hochrisikokonstellationen detailliert zu beschreiben und empirisch den konkurrenten und prädiktiven Wert von Risikoindizes empirisch zu bestimmen.
