Zusammenfassung

Adipositas und Diabetes mellitus Typ 2 (T2DM) nehmen global stark zu. Sie stellen weltweit ein wesentliches Problem der Volksgesundheit und Gesundheitsökonomie dar. Während die Chirurgie inzwischen in der Behandlung der morbiden Adipositas einen festen Stellenwert hat (bariatrische Chirurgie), kommt aktuell der Frage nach einer operativen Therapie eines alleinigen T2DM immer mehr Bedeutung zu (metabolische Chirurgie). Die möglichen operativen Therapieverfahren zeigen dabei eine unterschiedlich starke Beeinflussung des Gewichtes und der assoziierten Begleiterkrankungen. Die genaue Kenntnis der zur Verfügung stehenden operativen Therapien bei morbider Adipositas, deren Risiken und Anforderungen an den minimal-invasiven Chirurgen, deren Effektivität auch in Hinblick auf assoziierte Begleiterkrankungen und, soweit bekannt, deren Wirkmechanismen, sind eine entscheidende Voraussetzung für den gezielten Einsatz.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
