Komplikationsmanagement nach Lebertransplantation

Erhöhung der Patientensicherheit durch standardisiertes Vorgehen und interdisziplinäre Kooperation

Zusammenfassung

Nach Lebertransplantation ist das interdisziplinäre Komplikationsmanagement von großer Bedeutung. Unter dem Gesichtspunkt der derzeit dramatisch schlechten Organspendebereitschaft und der Priorisierung von schwerstkranken Patienten bei der MELD-basierten Allokation ist eine Zunahme von spender- und empfängerassoziierter Morbidität und Mortalität erkennbar. Zur frühzeitigen Erkennung von Problemen und der damit verbundenen erfolgreichen Komplikationsbehandlung ist ein strukturiertes und im interdisziplinären Team verbindlich festgelegtes Monitoringkonzept essenziell. Interdisziplinär gehaltene Visiten, Labor- und sonographische Perfusionskontrollen sind hierbei genauso wesentliche Bestandteile wie eine konsequente Infektionspropylaxe und moderne Immunsuppression. So sind arterielle Perfusionsstörungen unterschiedlicher Genese, Gallengangskomplikationen und komplizierte Flüssigkeitskollektionen in Abwägung aller vorhandenen Therapieoptionen und Erfahrungswerte individuell zu therapieren. Zusammenfassend lässt sich festhalten, dass der Erfolg einer Lebertransplantation in entscheidendem Maße vom Teamkonzept und der engen Kommunikation zwischen den beteiligten Fachabteilungen abhängt.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
