Protheseninfektionen im aortofemoralen Bereich und ihre Vermeidung

Zusammenfassung

Bei Protheseninfekten handelt es sich um schwerwiegende Komplikationen gefäßchirurgischer Eingriffe. Insbesondere im aortofemoralen Bereich gehen sie mit einer hohen Morbidität und Mortalität einher. Der nachfolgende Artikel behandelt das Thema der Protheseninfektion in der Gefäßchirurgie mit Schwerpunkt auf Erkennung und Vermeidung derselben und bietet dem Leser als CME-Beitrag auch einen Überblick über die Therapieoptionen bei vorliegendem Protheseninfekt. Die Diagnose einer Protheseninfektion kann nur in gemeinsamer Einschau der klinischen Präsentation, mikrobiologischer Befunde und bildgebender Verfahren gesichert werden. Monoinfektionen als Ursache von Protheseninfekten verlieren zugunsten von zunehmend häufigeren Mischinfektionen an Bedeutung, während Infektionen mit resistenten Erregern vermehrt diagnostiziert werden, wobei es unklar ist, ob eine sich ändernde Epidemiologie oder Fortschritte im Bereich der molekularmikrobiologischen Diagnostik dafür verantwortlich sind. In der Prävention hatte in den vergangenen Jahrzehnten die perioperative Antibiotikaprophylaxe einen besonderen Stellenwert, zunehmend wird diese Einzelmaßnahme durch weitere primärpräventive Hygienemaßnahmen im Sinne von Präventionsbündeln ergänzt. In vielen Fällen erfordert eine Infektion mit Involvierung der Prothese ihren Ausbau und In-situ- oder auch extraanatomische Revaskularisierungen. Bei der Formulierung eines Behandlungsplans müssen mehrere Faktoren, darunter nicht zuletzt der Allgemeinzustand des betroffenen Patienten, in Betracht gezogen werden. Auch der palliative Prothesenerhalt, insbesondere bei Patienten mit relevanten Komorbiditäten und hohem Operationsrisiko, gilt heute als vertretbare Behandlungsoption, vor allem unter Anwendung konservativer sowie lokalchirurgischer Maßnahmen wie der Vakuumtherapie in ausgewählten Fällen von Infektionen in unmittelbarer Umgebung einer Gefäßprothese.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
