Zusammenfassung

Hintergrund

Die Residualtumor(R)-Klassifikation beschreibt den Tumorstatus nach einer Therapie, d. h. bei Patienten mit Schilddrüsenkarzinomen vorwiegend nach einer chirurgischen Therapie. Das Ziel ist, den Erfolg der (chirurgischen) Therapie anzugeben, was das weitere therapeutische Vorgehen beeinflussen kann und relevante prognostische Aussagen erlaubt.

Fragestellung

Definition der R1-Resektion als auch des minimal invasiven extrathyreoidalen Wachstums von Schilddrüsenkarzinomen als häufige Voraussetzung einer R1-Resektion.

Ergebnisse

Darstellung der pathologischen Aufarbeitung und histopathologischen Beurteilung. Vorschlag zur Ergänzung und Erweiterung der seit 2017 gültigen TNM-Klassifikation (8. Auflage), um minimal invasive extrathyreoidale Schilddrüsenkarzinome systematisch klassifizieren zu können.

Schlussfolgerungen

Die dargestellten Definitionen (R1, minimal invasives extrathyreoidales Wachstum) in Verbindung mit der vorgeschlagenen Erweiterung der TNM-Klassifikation erlauben die prospektive wissenschaftliche Bewertung der biologischen Relevanz der beiden Parameter.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
