Zusammenfassung

Anaphylaxie ist eine akute systemische Überempfindlichkeitsreaktion mit den Symptomen der Soforttypallergie, die sich v. a. an Haut, Respirationstrakt, Herz-Kreislauf-System und Gastrointestinum manifestiert. Anaphylaktische Reaktionen verlaufen unterschiedlich schwer, manche führen zum Tod des Patienten. Von Insektengiftanaphylaxie sind etwa 3% der Allgemeinbevölkerung, von Nahrungsmittelüberempfindlichkeit etwa 2,6–3,2% betroffen; weitere häufige Auslöser sind Arzneistoffe. Die Symptome der Anaphylaxie sind charakteristisch, aber kein Symptom ist obligat – selbst Urtikaria fehlt in etwa 10% –, und jedes Symptom kann auch bei anderen Erkrankungen auftreten. Es kommen daher zahlreiche Differenzialdiagnosen in Betracht, und es ist nicht erstaunlich, dass Anaphylaxie oft nicht erkannt wird. Die Diagnose der Anaphylaxie wird anhand des Auftretens charakteristischer Symptome gestellt, insbesondere wenn diese im Zusammenhang mit der Exposition gegenüber einem potenziellen Auslöser auftreten. Sie wird wesentlich gestützt durch den Nachweis einer Mediatorfreisetzung im Reaktionsverlauf. Für die Routinediagnostik geeignet ist der Nachweis eines Anstiegs der Serumtryptasekonzentration über den individuellen Basiswert. Die korrekte Diagnose einer Anaphylaxie ist nicht nur für die Behandlung einer akuten Reaktion wichtig, sondern v. a. auch für die weitere allergologische Diagnostik und die erforderliche Langzeitversorgung des Patienten.
