Zusammenfassung
Die Graft-versus-Host-Disease (GvHD) stellt nach wie vor eine der Hauptkomplikationen nach allogener Stammzelltransplantation (SCT) dar und ist mitverantwortlich für die Morbidität, Mortalität und Einschränkung der Lebensqualität der Patienten nach SCT. Die wichtigste Primärprophylaxe zur Vermeidung einer GvHD ist die Wahl eines Spenders mit möglichst guter Übereinstimmung von Histokompatibilitätsmerkmalen. Die Basisprophylaxe der akuten GvHD, die bereits vor Transplantation gegeben wird, besteht aus Ciclosporin A, ggf. in Kombination mit Methotrexat. In der Therapie der akuten und chronischen GvHD sind in den letzten Jahren zahlreiche neue Behandlungsverfahren eingeführt worden. Für die Therapie der akuten und chronischen GvHD stellt z. B. die extrakorporale Photopherese (ECP) eine vielversprechende, nebenwirkungsarme Behandlungsmöglichkeit dar. Bei der chronischen GvHD hat der Einsatz von mTOR-Inhibitoren (Sirolimus, Everolimus) anstelle von Calcineurininhibitoren den Vorteil, keine Hauttumoren zu induzieren. Hilfreich für die Therapieentscheidung sind aktuell publizierte Behandlungsempfehlungen für die akute und chronische GvHD. Während die Pathophysiologie, die Stadieneinteilung und die Klinik der GvHD bereits in Teil 1 des Weiterbildungsartikels besprochen wurden, geht Teil 2 nun auf die Prognose, die Prophylaxe und die Therapie der GvHD näher ein.
Abstract
Graft-versus-host disease (GvHD) remains one of the major complications after allogeneic stem cell transplantation (SCT) and is responsible for morbidity, mortality and decrease in quality of life of patients after SCT. The most important preventive approach is the selection of a donor with best possible HLA compatibility between donor and recipient. Basic prophylaxis of acute GvHD begins already prior to transplantation and usually consists of cyclosporine with or without methotrexate. In the past few years, many new therapies have been introduced for the treatment of acute and chronic GvHD. Extracorporeal photopheresis (ECP), for example, represents a promising treatment option for acute and chronic GvHD with very few side effects. For chronic GvHD mTOR inhibitors (sirolimus, everolimus) may replace calcineurin-inhibitors with the advantage of not inducing malignant skin tumors. Guidelines are available ort he management of acute and chronic GvHD. While pathophysiology, classification and skin manifestations of GvHD have been already presented in the first part of this article, this second part covers the prognosis, prevention and treatment of GvHD.
