Die Ableitung zervikaler und okulärer vestibulär evozierter myogener Potenziale

Zusammenfassung

Die klinische Bedeutung der vestibulär evozierten myogenen Potenziale (VEMP) hat in den letzten Jahren zugenommen. VEMP sind inzwischen ein fester Bestandteil neurootologischer Untersuchungen zur Einschätzung des Funktionszustands der Otolithenorgane. Diese sind empfindlich auf niederfrequente akustische Stimuli. Bei einer Reizung kommt es zur Aktivierung der Rezeptoren in Sacculus und Utriculus. Über reflektorische Verbindungen können so myogene Potenziale unter der Voraussetzung einer tonischen Aktivierung der entsprechenden Muskeln erfasst werden. Der vestibulocollische (sacculucollische) Reflex verläuft von den Otolithenorganen über zentrale Verschaltungen zum ipsilateralen M. sternocleidomastoideus. Myogene Potenziale können hier mittels der sog. zervikalen VEMP (cVEMP) abgeleitet werden. Der vestibulooculäre Reflex kreuzt nach kontralateral zu extraokulären Augenmuskeln. Okuläre VEMP (oVEMP) werden periokulär, vorzugsweise über dem M. obliquus inferior abgeleitet. Für die Stimulation werden überschwellige Luftleitungsreize und vibratorische Knochenleitungsstimuli eingesetzt.
