Tissue-Engineering mit porösen Polyethylenimplantaten

Zusammenfassung

In der rekonstruktiven Gesichtschirurgie werden bereits Implantate aus porösem Polyethylen (PPE) genutzt, wobei aber die Verwendung im Rahmen von Rhinoplastiken weiterhin zu den kritischen Indikationen gehört. Die Biokompatibilität von PPE-Implantaten (Porengröße ~100–200 µm) kann durch Tissue-Engineering modifiziert werden: Beschichtungen mit Extrazellulärmatrixkomponenten, Wachstumsfaktoren, entzündungshemmenden Substanzen und vitalen Zellen sind möglich. Besonders geeignet für die präklinische Evaluation erscheinen gegenwärtig tierexperimentelle transparente Rückenhautkammermodelle. Bei Anwendung intravitalmikroskopischer Fluoreszenztechniken können das Einwachsen neugebildeter Blutgefäße und entzündliche Leukozyten-Endothelzell-Interaktionen im Detail unter Berücksichtigung der funktionell relevanten mechanischen Integration analysiert werden. Die präklinischen Daten deuten darauf hin, dass neuartige PPE-Implantate durch Tissue-Engineering derart in ihrer Biokompatibilität verbessert werden könnten, dass hierdurch die klinischen Anwendungsmöglichkeiten erweitert werden könnten.
