Psychische Komorbidität bei Tinnitus

Zusammenfassung

Tinnitus ist ein Phantomphänomen des Gehörs, für das ein Geräuschempfinden charakteristisch ist, ohne dass tatsächlich Geräuschquellen identifizierbar sind. Zum jetzigen Zeitpunkt sind seine Ursachen noch nicht vollständig geklärt. Der Schweregrad des Tinnitus hängt eher mit psychologischen und allgemeinen gesundheitlichen Faktoren als mit audiometrischen Parametern zusammen. Zusammen mit den limbischen Strukturen des ventralen Striatum bildet der präfrontale Kortex ein internes „Lärm-Cancelling-System“, das normalerweise unangenehme Geräusche einschließlich des Tinnitussignals ausblendet. Durch eine Störung dieses Durchgangswegs entsteht chronischer Tinnitus. Patienten mit chronischem Tinnitus zeigen erhöhte funktionale Verknüpfungen in den kortikolimbischen Passagen. Patienten, die wegen Tinnitus oder Hyperakusis den Arzt aufsuchen, leiden häufig unter psychiatrischer Komorbidität. Der klinisch tätige Arzt benötigt daher geeignete Screeningwerkzeuge, um psychiatrische Erkrankungen feststellen und diese multidisziplinär maßgeschneidert behandeln zu können.
