Hypoglykämischer Krampfanfall unter Propranolol

Zusammenfassung

Nach über 6 Monaten problemloser Behandlung mit Propranolol wegen eines Säuglingshämangioms erlitt ein 11 Monate altes Mädchen einen hypoglykämischen zerebralen Krampfanfall. Wahrscheinliche Erklärung ist eine Kombination aus vorausgehender reduzierter Nahrungsaufnahme bei Infekt, Verabreichung des Medikaments auf nüchternen Magen und anschließender Nahrungsverweigerung. Als Konsequenz aus dem Ereignis empfehlen wir, Propranolol erst gegen Ende einer Mahlzeit zu geben sowie die Verabreichung vorübergehend auszusetzen, wenn ein Kind bei 2 Mahlzeiten in Folge deutlich weniger als üblich isst.

Abstract

We recently treated a female infant with propranolol for infantile hemangioma. Treatment was effective and well-tolerated for 6 months; however, at the age of 11 months, the infant suffered hypoglycemia and concomitant seizure. This event could possibly be attributed to a number of factors: the child’s appetite was poor due to an ongoing infection, propranolol was administered on an empty stomach, and the child subsequently refused to eat. We thus introduced the following precautionary measures: propranolol should be administered at the end of meals, and treatment should be discontinued if the child eats poorly at two successive meals.
