CPM—Continuous Passive Motion: Behandlung verletzter und operierter Kniegelenke mit Mitteln der passiven Bewegung

Metaanalyse der gegenwärtigen Literatur

Zusammenfassung

Die Anwendung motorbetriebener Bewegungsschienen in der Kniegelenkchirurgie, das Prinzip „continuous passive motion“ (CPM), wird in der Literatur sehr kontrovers diskutiert. Daher sollten in einer Literaturrecherche alle Publikationen seit 1990 evaluiert werden. Dabei zeigte sich, dass Arbeiten mit negativen Ergebnissen häufig bereits vor dieser Zeit erschienen sind und weiterhin zitiert werden. In den medizinischen Datenbanken subito-doc.de, medscape.com, medica.de und zbmed.de fanden sich 230 Publikationen welche die Suchbegriffe „CPM“, „continuous passive motion“ und „Arthromot“ enthielten. Gleichzeitig wurden Autoren gesucht, die in anderen Arbeiten zitiert worden waren. Zur CPM-Therapie nach Kniegelenkeingriffen konnten 36 Arbeiten ausgewertet werden. Die Rolle von CPM in Bezug auf Bewegungsumfang, Schwellung, Krankenhausaufenthalt, Analgetikaverbrauch, Kostenanalyse, postoperative Manipulation, Wundheilungsstörung und thromboembolische Komplikationen wurde bewertet. Trotz widersprüchlicher Resultate der teils retrospektiven, teils prospektiven, randomisierten und teils verblindeten Studien lässt sich bisher nur ein Trend zu verbesserten Behandlungsergebnissen feststellen. Dies erfordert neue klinische Studien zur Erarbeitung objektiver Leitlinien für die Anwendung von CPM nach Kniegelenkeingriffen.
