Versorgung von bikondylären Tibiakopf-Frakturen mit einem unilateral platzierten winkelstabilen Plattensystem

Zusammenfassung

Hintergrund

Die kombinierte Plattenosteosynthese (4,5-mm-Hauptplatte und 1/3-Rohr-Antigleitplatte) gilt als Standardverfahren zur Stabilisierung bikondylärer Tibiakopf-Frakturen. Ziel dieser Studie war es, Frühergebnisse nach der Versorgung bikondylärer Tibiakopf-Frakturen mit einem unilateralen winkelstabilen Plattensystem zu gewinnen.

Material und Methoden

Im Rahmen einer multizentrischen Studie wurden Patienten mit einer AO/ASIF-41-C-Fraktur, die mit einem unilateralen, winkelstabilen Plattenfixateur stabilisiert worden war, untersucht. Der Nachuntersuchungszeitraum lag bei 11 bis 13 Monaten. Hierbei wurden Fehlreposition, sekundärer Repositionsverlust und als Parameter für die klinische Untersuchung der Lysholm- und der Rasmussen-Score erhoben sowie retrospektiv ausgewertet. Die Analyse erfolgte mit Hilfe eines standardisierten Messsystems. Als primäre Fehlreposition bzw. als sekundärer Repositionsverlust wurde ein intraartikulärer Versatz ab 2 mm oder eine Fehlrotation von 5° und mehr in der Frontalebene gewertet.

Ergebnisse

Insgesamt wurden 68 Patienten mit 69 Frakturen, inklusive 14 offener Frakturen, eingeschlossen. Eine primäre Spongiosaplastik wurde bei 13 Patienten durchgeführt. Eine signifikante primäre Fehlreposition zeigte sich bei 13 Patienten. Von den 68 Patienten erschienen 62 (91%) zur letzten Follow-up-Untersuchung. Bei einem Patienten konnte keine Frakturheilung durch Versorgung mit dem unilateralen Plattenfixateur erreicht werden. Die Infektionsrate war mit 4 oberflächlichen und einem tiefen Infekt gering (6%). Bei 9 Patienten wurde ein sekundärer Repostitionsverlust beobachtet (15%). Ein Anhalt für Bandinstabilitäten ergab sich nicht. Die ermittelten Nachuntersuchungsscores von 54 Patienten zeigten in 47 Fällen ein gutes bis exzellentes Ergebnis für den Lysholm-Score (im Durchschnitt 87,2 von 100) sowie bei 44 Patienten für den Rasmussen-Score (im Durchschnitt 26,7 bei 5–48).

Schlussfolgerung

Aufgrund niedriger Infektionsraten und niedriger Knochenheilungsstörungen erscheint die unilaterale Stabilisierung bikondylärer Tibiakopf-Frakturen mit einem winkelstabilen Plattenfixateur speziell bei Frakturen mit höheren Weichteilschäden vorteilhaft. Primäre Achsenfehler zeigen jedoch mögliche Probleme bei der Anwendung der geschlossenen Reposition für Ungeübte.
