Wertigkeit der MRT in der präoperativen Diagnostik proximaler Humerusfrakturen vs. CT und konventionelles Röntgen

Zusammenfassung

Ziel

Die diagnostischen Aussagen von konventionellem Röntgen, Computer- (CT) und Magnetresonanztomographie (MRT) bei akuten proximalen Humerusfrakturen (PHF) sollen verglichen werden. Die Hypothese, dass eine präoperative MRT therapierelevante Zusatzinformationen liefert, wird geprüft.

Material und Methode

Dreißig dislozierte PHF wurden mit Röntgen, CT und MRT untersucht. An jedes Verfahren wurden die gleichen Fragen zur Frakturanalyse gestellt und deren Aussagen verglichen. Die MRT-Bilder wurden additiv nach muskulotendinösen Begleitverletzungen und prognostischen Parametern für die Kalottenperfusion beurteilt.

Ergebnisse

Bei 7/30 PHF konnte im Röntgen die Fragmentzahl nicht korrekt ermittelt werden. Die prognostisch wesentliche posteromediale metaphysäre Extension sowie der Kaput-Diaphysen-Winkel wurden im Röntgen signifikant größer gemessen als in der CT/MRT. Relevante Zusatzinformationen lieferte die MRT bei 7/30 Rotatorenmanschettenrupturen und der in 11/30 Fällen abgrenzbaren medialen Perioststruktur.

Schlussfolgerung

Das Röntgen stellt die Basisdiagnostik für die PHF dar, die allerdings einer Erweiterung durch eine Schnittbilddiagnostik bedarf. Die MRT kann die gleichen Informationen zur Frakturmorphologie wie die CT sowie therapierelevante Aussagen über die Rotatorenmanschette und Kalottenperfusion liefern.
