Arthroskopische Bizepssehnentenodese mit isometrischer Sehnenrefixation

Die Technik der arthroskopischen Bizepssehnentenodese sollte zum operativen Repertoire eines Schulterchirurgen gehören, da die Indikationen vielfältig und somit relativ häufig sind. Das Indikationsspektrum reicht von mechanisch instabilen SLAP-Läsionen, isolierten Verletzungen des Bizepssehnenpulleys mit resultierender Subluxation oder Luxation der langen Bizepssehne über die therapieresistente Bizepssehnentendinitis und Partialrupturen bis hin zur anteriosuperioren Rotatorenmanschettenruptur [1, 2, 5]. Konkurrierend steht der Tenodese immer die proximale Tenotomie gegenüber, die wohl im Vergleich weniger funktionelle Defizite als eher kosmetische Einbußen nach sich zieht. Ein „evidenzbasierter“ Vergleich beider Techniken ist allerdings anhand der aktuell verfügbaren Literatur nicht möglich [3]. Bei Patienten unter 50 Jahren wird im eigenen klinischen Vorgehen aufgrund des kosmetischen Anspruchs bevorzugt die Tenodese der langen Bizepssehne angewandt.
