Depressive Störungen bei körperlich Kranken

Zusammenfassung

Krankheitswertige depressive Störungen liegen im Durchschnitt bei etwa 15% aller Menschen mit einer körperlichen Erkrankung vor, bei Patienten mit schwereren und chronischen Erkrankungen liegt die Prävalenzrate für depressive Störungen bei etwa 30–40%, wobei jeweils etwa die Hälfte der Prävalenz schwere bzw. leichtere depressive Syndrome betrifft. Eine möglichst präzise Diagnostik sowie eine frühzeitige und sachgerechte Behandlung depressiver Störungen bei körperlichen Erkrankungen erhöht die Lebensqualität der Patienten, mindert aber vermutlich auch einige der negativen Auswirkungen auf den Verlauf der somatischen Erkrankung, wobei ein direkter Einfluss auf die Prognose bisher nicht überzeugend gezeigt werden konnte. Auch moderne medizinische Behandlungsmaßnahmen können zur Entstehung depressiver Störungen führen. Ausmaß und Konsequenzen einer Komorbidität von somatischer Erkrankung und depressiver Störung, ihre sozioökonomischen Implikationen, aber auch die vorhandenen effizienten therapeutischen Möglichkeiten stehen in erstaunlichem Kontrast zu einer in der medizinischen Routineversorgung körperlicher Kranker nach wie vor mangelhaften Diagnostik und Therapie depressiver Störungen.
