Blutungsrisiko unter SSRI-Behandlung

Zusammenfassung

Eine Behandlung mit selektiven Serotoninwiederaufnahmehemmern (SSRI) erhöht das Risiko einer gastrointestinalen (GI-)Blutung. Durch eine Kombinationsbehandlung mit nichtsteroidalen Antirheumatika (NSAR) wird diese Blutungsneigung noch weiter gesteigert. Besondere Vorsicht gilt auch bei Patienten mit thrombozytenaggregationshemmender Medikation, einer bekannten Blutungsneigung sowie vorangegangenen GI-Läsionen. Blutungsereignisse außerhalb des GI-Traktes, wie intrazerebrale Blutungen, werden unter SSRI-Behandlung nicht gehäuft registriert. Dies betrifft auch die Kombination von SSRI mit Cumarinen oder mit Acetylsalicylsäure (ASS). Ärzte müssen das erhöhte Blutungsrisiko bei der Behandlung mit SSRI kennen und ihre Patienten darüber aufklären. Hochrisikopatienten müssen engmaschig kontrolliert werden, und es sollte ein SSRI mit geringem Interaktionspotenzial ausgewählt werden. Auch die Verschreibung von Gastroprotektiva und eine Umstellung der antidepressiven Medikation sind im Einzelfall zu erwägen. Wir geben eine Literaturübersicht und praktische Empfehlungen.
