Innovative Versorgungsstrategien in der Gerontopsychiatrie und -psychotherapie

Zusammenfassung

Hintergrund

Der demographische Wandel wird die gerontopsychiatrische Versorgung sowohl im Hinblick auf den Bedarf als auch hinsichtlich der Ressourcen vor neue Aufgaben stellen. Veränderungen in der Mobilität und die Häufigkeit an Komorbidität sowie die im Allgemeinen notwendige Hilfe im persönlichen Alltag stellen eine besondere Herausforderung an die interdisziplinäre Medizin mit dem Schwerpunkt Alterserkrankungen und es besteht die Notwendigkeit multiprofessioneller gerontopsychiatrischer und -psychotherapeutischer Behandlungsstrategien, die aufsuchende medizinische und psychosoziale Interventionen einschließen.

Fragestellung

Aktuelle Studien untersuchen mögliche Versorgungsstrategien für alte Menschen mit einer Indikation zur gerontopsychiatrischen und -psychotherapeutischen Behandlung.

Material und Methode

Es wird eine Auswahl aktueller Studien mit vorwiegend randomisiert-kontrolliertem (RCT) Design zur gerontopsychiatrischen und -psychotherapeutischen Behandlung von Patienten im höheren Lebensalter ausgewählt, um das Spektrum der gegenwärtigen Forschung aufzuzeigen.

Ergebnisse

Versorgungsstrategien, die einen niedrigschwelligen und mehrstufigen Zugang zu immer spezialisierteren gerontopsychiatrischen und -psychotherapeutischen Behandlungen ermöglichen und im hausärztlichen Versorgungssektor angeboten werden, zeigen einen signifikanten Effekt bei der Depressionsbehandlung. Die aufsuchende Therapie führt zur klinisch signifikanten Besserung bei alten Menschen und fehlendem Zugang oder fehlender Inanspruchnahme der psychiatrischen Therapie. Die telemedizinische gerontopsychotherapeutische Behandlung kann genau wie das klassische Therapiesetting zu signifikanten Verbesserungen der psychischen Symptome führen und weißt in einer Studie einen anhaltenden Effekt auf.

Schlussfolgerung

Die Diagnose und Behandlung alter Menschen mit psychischen Erkrankungen erfordert die Entwicklung und Implementierung integrierter Versorgungsstrategien für psychisch und körperlich Erkrankte im höheren Lebensalter. Diese sollten in enger Kooperation mit der primären, nämlich hausärztlichen Versorgung in niederschwelligen und stufenweise spezialisierteren Behandlungskonzepten umgesetzt werden. Telemedizinische Behandlungsstrategien sind auch im höheren Lebensalter vielversprechend und könnten eine breite Zugänglichkeit einer notwendigen spezialisierten gerontopsychiatrischen und -psychotherapeutischen Behandlung gewährleisten.
