Zusammenfassung

Eine Depression mit Erstmanifestation im höheren Lebensalter wird als „Altersdepression“ („late-onset depression“, LOD) bezeichnet. Die Symptomatik der LOD unterscheidet sich in vielen psychopathologischen Dimensionen nicht grundsätzlich von einer Depression in anderen Lebensphasen; typisch sind jedoch oft ausgeprägtere kognitive Defizite. Die LOD geht mit einem erhöhten Risiko für die Entwicklung einer Demenz einher. Bildgebungsstudien zeigen Volumenminderungen der grauen und durch Gefäßverschlüsse verursachte Läsionen der weißen Substanz. Das Auftreten einer Depression bei Gefäßschädigungen des Gehirns wird auch als „vaskuläre Depression“ bezeichnet. Die Diagnostik umfasst eine ausführliche Anamnese und Erhebung des psychopathologischen Befundes, körperliche Untersuchung, Labordiagnostik, EEG, EKG und MRT des Kopfes sowie eine Erfassung der kognitiven Defizite mittels neuropsychologischer Testung. Psychotherapie stellt eine wirksame Therapieoption dar. Pharmakologisch sind Serotoninwiederaufnahmehemmer die Therapie der ersten Wahl.
