Juveniler Schlaganfall – was gibt es zu beachten?

Zusammenfassung

Ein Schlaganfall bei jungen Erwachsenen stellt alle Beteiligten vor besondere diagnostische und therapeutische Herausforderungen. Etwa 15 % aller Schlaganfälle treten bei jungen Patienten auf. Die Symptome werden in der Notaufnahme häufig verkannt, sodass die Diagnose und damit auch der Zugang zu möglichen revaskularisierenden Behandlungsoptionen oft verzögert erfolgt. Die Ursachen sind oft heterogen und erfordern zum Teil spezifische Untersuchungen. Die steigende Prävalenz klassischer vaskulärer Risikofaktoren bei jungen Erwachsenen führt zu einer Zunahme der Inzidenz von Hirninfarkten, was dem rückläufigen Trend der allgemeinen Schlaganfallinzidenz entgegenläuft. Kardiogene und mikroangiopathische Ätiologien sind am häufigsten, des Weiteren kommen gegenüber älteren Patienten Dissektionen der Halsgefäße häufiger vor. Zu den speziellen Ursachen gehören Drogenkonsum, Vaskulitiden und Arteriopathien wie das reversible Vasokonstriktionssyndrom oder das posteriore reversible Enzephalopathiesyndrom. Intravenöse Lysetherapie und mechanische Thrombektomie sind aktuellen Studien zufolge bei jungen Patienten sicher und effektiv.
