Moderne sonographische Methoden zur Quantifizierung von Gewebeperfusion

Zusammenfassung

Die Perfusion stellt einen entscheidenden Vitalitätsparameter z. B. von Myokard, Hirn- oder Nierengewebe dar. In malignen Tumoren besitzt die Perfusion besondere Bedeutung in deren differenzialdiagnostischer und prognostischer Beurteilung. Mit der Erprobung pro- oder antiangiogener Therapieansätze werden weitere Anforderungen an die funktionelle Bildgebung zur quantitativen Diagnostik der Gewebevaskularität gestellt.
Dieser Beitrag befasst sich mit sonographischen Verfahren zur Detektion der Gewebemikrozirkulation, die über die etablierte Beurteilung größerer Gefäße durch die konventionelle Dopplersonographie hinausgehen. Der Schwerpunkt liegt dabei auf der Beschreibung der kontrastmittelunterstützten Sonographie, die durch quantitative Analyse von Wiederanflutungskinetiken der Mikrobläschen eine umfassende Beschreibung der Gewebeperfusion erlaubt. In experimentellen Untersuchungen kann hierdurch die Veränderung von Tumorvaskularität im zeitlichen Verlauf, z. B. beim Monitoring antiangiogener Therapien, untersucht werden. Weitere Einsatzgebiete sind die Messung der Myokard-, Hirn- und Nierenperfusion. Neue Ansätze zur Optimierung des zugrundeliegenden mathematischen Modells und neue Entwicklungen der Ultraschalltechnik werden diskutiert.
