Simultane Ganzkörper-PET-MRT in der pädiatrischen Onkologie

Mehr als nur Strahlenersparnis?

Zusammenfassung

Die bildgebende Diagnostik spielt in der pädiatrischen Onkologie eine zentrale Rolle für die Diagnose, die Therapieplanung und die Nachsorge solider Tumoren. Der aktuell bildgebende Standard in der pädiatrischen Onkologie sieht – abhängig von der vorliegenden Tumorentität – eine Kombination mehrerer radiologischer und nuklearmedizinischer Verfahren vor. Die Einführung der simultanen Positronenemissionstomographie(PET)-Magnetresonanztomographie (MRT) hat neuartige Möglichkeiten der Diagnostik in der pädiatrischen Onkologie eröffnet. Dabei kombiniert dieses neue Verfahren die exzellente anatomische Genauigkeit der MRT mit metabolischen Informationen der PET. In ersten klinischen Studien wurden die technische Durchführbarkeit sowie mögliche diagnostische Vorteile der kombinierten PET-MRT gegenüber alternativen Methoden aufgezeigt. So kann die Anwendung der kombinierten PET-MRT zu einer Reduktion der Strahlenexposition im Vergleich zur PET-Computertomographie um bis zu 70 % gesenkt werden. Zudem wurde gezeigt, dass die Anwendung der PET-MRT zu einer Reduktion der Anzahl notwendiger Untersuchungen führen kann. Aufgrund der geringen Verfügbarkeit stellt die kombinierte PET-MRT aktuell kein Routineverfahren dar. Dennoch hat diese neuartige Modalität das Potenzial, sich zum bildgebenden Referenzstandard in der pädiatrischen Onkologie zu entwickeln. Im vorliegenden Übersichtsbeitrag werden anhand der verfügbaren Literatur zentrale Aspekte der pädiatrisch onkologischen PET-MRT-Bildgebung dargestellt. Zusätzlich werden typische Anwendungsfälle der pädiatrischen PET-MRT präsentiert.
