Sentinel-Lymphadenektomie beim Prostatakarzinom

Erfahrungen nach über 800 Eingriffen

Zusammenfassung

Bei welchen Prostatakarzinompatienten Lymphknoten entfernt und welche Regionen mit einbezogen werden sollten, wird kontrovers diskutiert. Die meisten Urologen verlassen sich auf Nomogramme und verzichten daher beim Low-risk-Prostatakarzinom auf die Lymphadenektomie. Inzwischen häufen sich Untersuchungen, die zeigen, dass Lymphknotenmetastasen auch beim Low-risk-Prostatakarzinom vorkommen und vorwiegend außerhalb des Standarddissektionsgebiets liegen.
Die Ergebnisse der Sentinel-Lymphadenektomie (SLNE) bei den ersten 860 Patienten zeigen, dass Lymphknotenmetastasen wesentlich häufiger sind als von A.W. Partin prognostiziert. Mit der SLNE findet man Lymphknotenmetastasen außerhalb der Region der Standard- und der extendierten Lymphadenektomie.
Da präoperativ nicht sicher ist, welcher Patient Lymphknotenmetastasen hat, sollte man sich für die Lymphadenektomie entscheiden. Dabei könnten Patienten mit positiven Lymphknoten auch in kurativer Hinsicht von der Kombination aus Sentinel- und extendierter Lymphadenektomie profitieren.
