Die kniegelenknahe Focal-dome-Osteotomie

Zusammenfassung

Die Focal-dome-Osteotomie (FDO) ist eine zylindrische Osteotomie, deren korrespondierende Knochenenden ineinander um das Zentrum der Deformität gleiten. Hierdurch kann eine vollständige, achsengerechte Korrektur ohne unerwünschte Translationsabweichungen erreicht werden. Die FDO ermöglicht eine hohe intraoperative Justierbarkeit, einen optimalen Knochenkontakt sowie eine hohe Primärstabilität.
Ähnlich wie bei „Opening-wedge-“, „Closing-wedge-“ und „Neutral-wedge-Keilosteotomien“ können FDO als neutrale, öffnende und schließende Osteotomien durchgeführt werden. Aufgrund der domförmigen Osteotomie sind öffnende FDO unter Erhalt des Knochenkontaktes und schließende FDO ohne Verlust von Knochensubstanz möglich. Interligamentäre FDO können durch Valgisierung gleichzeitig zu einer Straffung des Knieinnenbandes führen. Infratuberositäre FDO beeinflussen üblicherweise die Funktion des Patellofemoralgelenks nicht und minimieren das Risiko intraartikulärer Frakturen. Sofern das gelenknahe Fragment eine ausreichende Länge aufweist, ist eine Stabilisation mit einem Verriegelungsnagel möglich. Auch für höhergradige Fehlstellungen in der Frontal- und Sagittalebene stellt die kniegelenknahe FDO eine hervorragende Therapieoption dar.
