Klinische Untersuchung bei patellofemoralen Problemen

Zusammenfassung

Patellofemorale Schmerzen und Instabilität sind mit über 25% aller sportassoziierten Beschwerden am Kniegelenk ein häufiges Krankheitsbild [4]. Trotzdem bereitet die Analyse dieser Probleme offensichtlich Mühe. Sammeldiagnosen wie „patellofemorales Schmerzsyndrom“, „vorderer Knieschmerz“ oder „Chondromalazie“ sind nicht selten [22]. Die Schwierigkeit liegt zum einen in der komplexen Pathomechanik des Patellofemoralgelenk, da die Patella im Extensorenapparat zwischen Becken, Femur und Tibia integriert ist und damit Einflüssen von Hüfte bis Fuß ausgesetzt ist. Zum anderen werden unterschiedliche klinische Zeichen und Funktionstests verwendet mit z. T. fehlender Quantifizierbarkeit und zweideutiger Interpretationsmöglichkeit. Das vorliegende Manuskript fasst daher die bekannten Zeichen und Funktionstests zusammen, beurteilt sie anhand der Literatur und ordnet sie in ein praktisches Untersuchungsschema ein. Dazu wurde die Literatur über Medline und spezielle Lehrbücher betreffend patellofemoraler Erkrankungen herangezogen. Es konnten 43 Zeichen und Tests ausfindig gemacht werden. Der Untersucher kann ohne spezielle Hilfsmittel 18 davon auf einer Nominal- und 15 auf einer Ordinalskala quantifizieren. Damit liefert dieses Manuskript dem Leser ein standardisiertes Untersuchungsschema mit einer Sammlung von quantifizierbaren Befunden, welche immer wichtiger werden, um eine adäquate Diagnostik und Therapie zu rechtfertigen.
