Implantatkomplikationen in der Schulterarthroskopie

Zusammenfassung

Mit der zunehmenden Zahl der Schulterarthroskopien steigt auch das Komplikationspotential durch den Einsatz von Nahtankern, welche hauptsächlich in der Rotatorenmanschettenrekonstruktion und der Schulterstabilisierung ihre Anwendung finden. Das Hauptproblem ist hierbei die Fehlpositionierung des Ankers. Die Folgen der Fehlpositionierung von Implantaten sind neben persistierenden Beschwerden häufig auch die Entwicklung von Knorpelschäden und/oder das Versagen der Rekonstruktion, welches eine erneute chirurgische Intervention erfordern kann. Insbesondere bei osteoporotischem Knochen ist ein Implantat zu wählen, welches eine ausreichende Primärstabilität bietet. Des Weiteren steigt mit der Menge der Implantate die Möglichkeit einer Fadenkomplikation. Hier kann eine Beschädigung des Nahtmaterials oder ein Ausfädeln des Fadens aus dem Anker dazu führen, dass der Anker unbrauchbar wird. Ein Verwickeln der Fäden führt im schlimmsten Fall zum frühzeitigen Abbruch der Arthroskopie und einem Verfahrenswechsel auf ein offenes Vorgehen.
Bioresorbierbare Implantate bieten im Vergleich zu Metallimplantaten einige Vorteile in Bezug auf eine mögliche Revisionsoperation und postoperative MRT-Kontrollen. Neben wirtschaftlichen Erwägungen mit erhöhten Implantatkosten sollten aus medizinischer Sicht bei biodegradierbaren Implantaten „Ankerlocherweiterungen“ und ein möglicherweise erhöhtes Versagen gegenüber Metallimplantaten in die Entscheidung der Implantatwahl mit einfließen. Eine seltene, aber nicht zu vergessende Komplikation ist die Implantatunverträglichkeit.
