Risikofaktoren für eine fehlgeschlagene Infektsanierung nach periprothetischer Hüftprothesenspätinfektion

Zusammenfassung

Hintergrund

Trotz aufwendiger Sanierungskonzepte sind Infektrezidive insbesondere bei chronischen Infekten von Hüftprothesen keine Seltenheit. Ziel der retrospektiven Studie war die Identifizierung von Faktoren, die eine Sanierung verhindern und ein Rezidiv begünstigen.

Material und Methoden

In die Studie wurden 40 Patienten mit periprothetischen Hüftprothesenspätinfektionen eingeschlossen, die zwischen 2006 und 2008 mit dem Ziel der Infektsanierung behandelt wurden. Die Infektsanierung erfolgte anhand eines standardisierten Behandlungsregimes mit zwei- und mehrzeitiger Hüftprothesenreimplantation nach erfolgreicher Keimeradikation. Eine klinische Verlaufskontrolle wurde nach 2 Jahren durchgeführt.

Ergebnisse

Achtzehn Patienten (45%) waren nach erfolgreicher Eradikation und Reimplantation einer Prothese rezidivfrei, 5 (12,5%) erlitten nach primär erfolgreicher Revisionsprothese ein Infektrezidiv. Bei 17 Patienten (42,5%) musste primär aufgrund eines nicht sanierbaren Befundes auf ein Alternativverfahren gewechselt werden. In der Gruppe der Patienten mit Infektrezidiv oder nicht sanierbarem Befund ließen sich signifikant häufiger resistente Keime nachweisen (p = 0,001), insbesondere methicillinresistenter Staphylococcus aureus (MRSA), methicillinresistenter Staphylococcus epidermidis (MRSE) und hochresistente Pseudomonaden. Diese Gruppe besaß im Vergleich zu rezidivfreien Patienten einen signifikant schlechteren American-Society-of-Anesthesiologists(ASA)-Score (p = 0,002). Die Anzahl operativer Eingriffe war in dieser Gruppe signifikant höher.

Schlussfolgerung

Ein schlechter Allgemeinzustand sowie resistente Infekterreger sind als Hauptrisikofaktoren für ein Infektrezidiv nach Prothesenreimplantation anzusehen. Somit ist bei polymorbiden Patienten mit resistenter Keimlage ein anderes Behandlungskonzept anzustreben.
