Differenzialindikationen aktueller Endoprothesensysteme der Schulter

Zusammenfassung

Moderne Schulterendoprothesen adaptieren sich modular an die von der Anatomie vorgegebene Größe, die Inklination, den posterioren Offset und die Retrotorsion der Schulter. Verwendet werden die Cup-Prothese als Oberflächenersatz, schaftfreie Prothesen mit metaphysärer Verankerung, anatomische Schaftprothesen mit unterschiedlich langen Schäften und nicht zuletzt die inverse Prothese. Hauptindikationen sind die primäre Omarthrose, die posttraumatische und die rheumatoide Arthritis, die avaskuläre Humeruskopfnekrose, die Instabilitätsarthrose und die Defektarthropathie.
Anatomische Hemiprothesen sollten nur verwendet werden, solange die Pfanne intakt ist, weil Totalprothesen bei einer arthrotisch veränderten Pfanne das bessere Ergebnis haben. Konventionelle Schäfte werden überwiegend zementiert. Zementierte, auf der Rückseite konvexe PE-Glenoide sind als Standard anzusehen. Die für posttraumatische Indikationen entwickelten schaftfreien Prothesen können bei guter Knochenqualität häufig einen konventionellen Schaft ersetzen. Die inverse Prothese wurde für die Defektarthropathie entwickelt. Ihre Verwendung als Revisionsimplantat bedingt höhere Komplikationen und eine kürzere Standzeit.
