Zusammenfassung

Hintergrund

Der M. Osgood-Schlatter stellt eine häufige, temporäre Schmerzerkrankung meist nur eines Kniegelenks bei sportlichen Adoleszenten dar. Ursächlich ist die starke Beanspruchung der tibialen Apophyse.

Ergebnisse

Die eigene Studie an 29 konsekutiven Patienten (∅ 12,7 Jahre, 77 % männlich, 67 % nichtdominante Seite betroffen, 4‑mal beidseits, immer Schmerz nach sportlicher Belastung, hohe sportliche Aktivität in Fußball, Basketball, Turnen/Ballett, 41 % Rectusverkürzung) zeigt, dass die Diagnose rein klinisch ausreichend ist.

Diskussion

Bildgebung ist nur bei gerechtfertigtem Verdacht auf eine konkurrierende Erkrankung gerechtfertigt, da sie uneinheitlich ist und keine Hinweise auf Therapie oder Prognose zulässt. Die Betroffenen modifizieren ihre Beanspruchung schmerzadaptiert. Spätfolgen liegen allenfalls in der möglichen, auf Druckbelastung schmerzhaften Vorwölbung der Tuberositas tibiae. Problematisch sind restriktive Empfehlungen an die in aller Regel aktiven bewegungsfreudigen Patienten, obwohl zur Sportkarenz keine Daten existieren.
