„Black esophagus“

Phänomenologische und pathomorphologische Analyse

Zusammenfassung

Hintergrund

Der „black esophagus“ ist ein äußerst seltenes Krankheitsbild, das aufgrund seiner hohen Letalität eine wichtige Differenzialdiagnose der unklaren oberen gastrointestinalen Blutung darstellt. Seit der Erstbeschreibung 1967 wurden lediglich 112 Fälle vorwiegend in kasuistischer Form in der Literatur veröffentlicht. In nur 7 Fällen wurde die Diagnose des Black esophagus bisher autoptisch gestellt. Makroskopisch weist die ösophageale Schleimhaut eine schwarze Verfärbung auf, die sich histologisch als vollständige Nekrose der Mukosa darstellt. Der Pathomechanismus ist noch immer unklar. Es scheint eine heterogene Ätiologie zugrunde zu liegen, die auf der Kombination einer Ischämie der Schleimhaut und einer direkt reizenden Wirkung des Mageninhalts beruht sowie auf verringerten mukosalen Schutzmechanismen und erniedrigter Regenerationsfähigkeit des mukosalen Gewebes. Insbesondere die Stoffwechselentgleisung Ketoazidose ist hinsichtlich ihrer ursächlichen Bedeutung vermehrt in den Fokus gerückt.

Ziel der Arbeit

Die vorliegende Charakterisierung soll Hinweise auf den Entstehungsmechanismus eines Black esophagus geben.

Material und Methoden

In dieser retrospektiven Obduktionsstudie wurden 31 Black-esophagus-Fälle, die innerhalb von 5 Jahren an den beiden rechtsmedizinischen Instituten in Berlin obduziert wurden, makroskopisch, histologisch und toxikologisch charakterisiert.

Ergebnisse

Die Studie zeigt, dass es offensichtlich einen engen Zusammenhang zwischen chronischem Alkoholabusus, der Entwicklung einer Ketoazidose und der Entstehung eines Black esophagus gibt.
