HIV encephalitis with subcortical tau deposition: imaging pathology in vivo using F-18 THK 5117

A 70-year-old widow of 4 years presented with memory complaints for the past 4 months, apraxia, and bladder and bowel incontinence. As a part of an ongoing study the patient underwent both F-18 FDG positron emission tomography (PET) and F-18 THK5117 PET. There was frontal, parietal, and temporal hypometabolism on F-18 FDG PET (a) while F-18 THK 5117 study showed increased TAU binding in the periventricular and deep white matter regions (b). MRI showed hyperintensities in the periventricular and deep white matter (c). The patient was subsequently found to be HIV positive on ELISA with a CD4 count of 124. A working diagnosis of HIV encephalitis (HIVE) was made. …
