The football sign is a large oval lucency extending along the craniocaudal long axis of the abdomen on supine abdominal radiograph. Football sign represents a massive pneumoperitoneum distending the peritoneal cavity. While patient is lying supine, the free abdominal air accumulates anteriorly delineating the parietal peritoneum and thereby resembling an American football shape [1, 2] (Figs. 1, 2, 3, and 4).
