Zusammenfassung

Die Keimstrang/Stromatumoren sind neben den Keimzelltumoren die wesentlichen endokrinen Tumoren des Hodens. Hinzu kommt das primäre Karzinoid, dessen Beziehung zu den Teratomen aber bis heute unklar ist. Stromatumoren sind insgesamt sehr selten, endokrin aktiv sind mindestens 10 bis 20%. Unter den Stromatumoren sind praktisch lediglich Leydig-Zell-Tumoren und Sertoli-Zell-Tumoren von Bedeutung. Endokrine Störungen gehen meist von den Leydig-Zell-Tumoren aus (Gynäkomastie, Pubertas praecox), sie sind bei den übrigen Stromatumoren weniger häufig, können aber grundsätzlich vorkommen. Der großzellige kalzifizierende Sertoli-Zell-Tumor ist meist Teil einer übergeordneten Störung (z. B. Peutz-Jeghers-Syndrom). Für die immunhistologische Charakterisierung von Bedeutung sind Inhibin, Calretinin, Zytokeratine, Melan-A, CD-99, Proliferationsmarker, der Androgenrezeptor und p53. Konventionelle Morphologie und immunhistologisches Färbemuster überlappen sich, unklare Fälle sollten deshalb grundsätzlich spezialisierten Zentren zugeleitet werden.
