Zusammenfassung

Die Graft-versus-Host-Erkrankung (GvHD) ist eine sehr häufige Komplikation nach allogener Stammzelltransplantation mit einer Inzidenz von 35–50%. Nach Zeitpunkt des Auftretens, Art der klinischen Symptome, klinischem Verlauf und histologischem Befund wird zwischen einer akuten und chronischen GvHD differenziert. Beide manifestieren sich bevorzugt im Bereich der Haut, der Schleimhäute, des Gastrointestinaltrakts, der Leber und seltener der Lunge. Da die klinischen Symptome insbesondere der akuten GvHD unspezifisch sind, ist für die Abgrenzung von floriden Infekten und Medikamenten-assoziierten Nebenwirkungen die Histologie von besonderer Bedeutung. Diese Übersicht stellt die morphologischen Kriterien für die Diagnostik der GvHD der Haut, der Leber und des Gastrointestinaltrakts vor dem Hintergrund der wesentlichen Differenzialdiagnosen vor.
