Myofasziale Dysfunktion in der S2k-Leitline Spezifischer Kreuzschmerz

Zusammenfassung

Aus der S2k-Leitlinie Spezifischer Kreuzschmerz wird die funktionelle Entität „myofasziale Dysfunktion“ als weitere Ursache für den spezifischen Kreuzschmerz vorgestellt. Myofasziale Dysfunktionen sind Ursache und Folge von Fehlbelastungen des Bewegungssystems. Durch Modifikation einströmender Afferenzen sowohl propriozeptiven als auch nozizeptiven Ursprungs kommt es zur Veränderung der Bewegungs- und Haltungssteuerung (geänderte motorische Muster). Die Diagnostik der myofaszialen Dysfunktion erfolgt durch gezielte, auch anatomisch definierte klinische Funktionstests. Diese sollen in einen manualmedizinischen bzw. funktionellen orthopädischen Untersuchungsgang integriert sein und anschließend strukturiert bewertet werden. In der Therapie werden in erster Linie neuroreflektorische und biomechanische Mechanismen genutzt.
