Das „feuchte“ trockene Auge

Zusammenfassung

Das trockene Auge ist eine multifaktorielle Erkrankung der Tränenflüssigkeit und der Augenoberfläche, die zu Beschwerden, Sehstörungen und Tränenfilminstabilität mit möglicher Schädigung der Augenoberfläche führt. Im Wesentlichen führen zwei Mechanismen zum Bild des trockenen Auges: eine qualitative Reduktion der Tränenproduktion und eine erhöhte Verdunstung des Tränenfilms. Das evaporativ trockene Auge führt zu einer Irritation mit sekundärer Tränenhypersekretion, die dann das klinische Bild eines „feuchten Auges“ für Untersucher und Patienten zeigen kann. Die Kenntnis und das Verständnis der zugrunde liegenden Pathologien und Differenzialdiagnosen ist wichtig, um diese sehr häufige Ursache einer Epiphora z. B. von Erkrankungen der ableitenden Tränenwege abgrenzen und um eine adäquate Therapie einleiten zu können.
