Zusammenfassung

Serielle Fallberichte von 3 männlichen Patienten mit Papillenschwellung und Visusminderung

Die Patienten entwickelten primär eine Visusminderung ein- bzw. beidseitig bei angiographisch und klinisch diagnostiziertem Papillenödem. Bei unauffälliger neurologischer und internistischer Abklärung bestätigte eine Serologie letztlich den Verdacht auf eine Papillitis bei akuter Syphilis.

Schlussfolgerung

Eine Papillitis mit Visusminderung kann Symptom einer Syphilis sein. Man sollte hier nicht nur an die bekannte plakoide Chorioretinitis bei Lues denken. Eine ausführliche Anamnese und diagnostische Maßnahmen sind zur Ursachenfindung unerlässlich. Zielführend sind meist auch das relativ junge Alter der Patienten, das männliche Geschlecht, sexuelle Orientierung sowie fehlende klassische Risikofaktoren für eine anteriore ischämische Optikusneuropathie (AION) oder neurologische Ursachen für eine randunscharfe geschwollene Papille wie eine intrakranielle Raumforderung (Stauungspapille).
