Einfluss der Kognition auf die Behandlungsergebnisse in der Geriatrie

Zusammenhang von MMSE und Zugewinn in den Alltagsaktivitäten

Hintergrund

Kognitive Defizite bzw. eine Demenz beeinflussen die Ergebnisse der Behandlung in der Geriatrie. Ziel der Untersuchung war es, diesen Zusammenhang näher zu beleuchten.

Patienten und Methoden

Dafür wurden Daten von 2527 Patienten aus dem Zeitraum 2006−2009 einbezogen. Analysiert wurde der Einfluss der Kognition, gemessen mit dem MMSE („mini mental status examination“) bei Aufnahme, auf die Verbesserung in den Alltagsaktivitäten, gemessen mit dem FIM („functional independence measure“).

Ergebnisse

Eine schlechtere Kognition geht mit einem geringeren funktionellen Status bei Aufnahme sowie bei Entlassung aus der Geriatrie einher. Patienten mit einer leichten kognitiven Beeinträchtigung von 20–26 Punkten im MMSE erreichen jedoch gleich gute Verbesserungen im FIM wie kognitiv intakte Patienten. In den Bereichen unterhalb eines MMSE von 20 Punkten sind die Verbesserungen im FIM im Mittel signifikant geringer: Im MMSE-Bereich von 10–19 Punkten erzielen 40 % und im MMSE-Bereich von 0–9 Punkten erreichen noch 30 % der Patienten einen größeren Zuwachs im FIM als der Durchschnitt der Patienten.

Schlussfolgerung

Patienten mit einem MMSE von 19 Punkten oder darunter sollten nicht generell von einer Behandlung in der Geriatrie ausgeschlossen, sondern es sollten individuelle Faktoren berücksichtigt werden.
