Anwendung und Therapiemonitoring bei entzündlich-rheumatischen Erkrankungen

Zusammenfassung

Glukokortikoide (GC) sind seit mehr als 70 Jahren bewährte medikamentöse Substanzen in der Rheumatologie. Sie wirken in hohen Dosen sehr rasch über ihre membranstabilisierenden Effekte. Die bei allen, auch sehr geringen Dosen auftretenden genomischen therapeutischen Effekte der GC beruhen zum großen Teil auf der Hemmung der Funktionen des Transkriptionsfaktors „nuclear factor kappa B“ (NFκB), der die Synthese von proinflammatorischen Mediatoren, Adhäsionsmolekülen und anderen regulatorischen Proteinen befördert. Die Indikation für den Einsatz von GC in hohen Dosen wird in der Rheumatologie immer dann gestellt, wenn es sich um eine lebensbedrohliche, gefährliche oder therapieresistente Situation handelt. Niedrigere, in der Regel oral verabreichte GC-Dosen finden ihre Anwendung v. a. bei der rheumatoiden Arthritis, Vaskulitiden und Kollagenosen. Bei ihrer Anwendung in der klinischen Medizin gilt das generelle Prinzip, die kleinstmögliche effektive GC-Dosis für die kürzest mögliche Zeit anzuwenden, um die therapeutische Wirkung der GC zu nutzen, ohne dass unakzeptabel ausgeprägte Nebenwirkungen in Kauf genommen werden müssen.
