Schmerztherapie bei ambulanten Operationen im Kindesalter

Zusammenfassung

Eine Vielzahl der Operationen im Kindesalter eignet sich für ein ambulantes Vorgehen. Die Vorteile sind bestechend>: Kostenreduktion, geringere Infektionsrate, Vermeidung einer Hospitalisation mit entsprechender psychischer Belastung, zeitnahe Rückkehr des Patienten ins gewohnte häusliche Umfeld. Als wesentliches Qualitätsmerkmal ambulanten Operierens gilt die Effizienz der perioperativen Schmerztherapie, wobei sich ein multimodales Vorgehen als sehr wirkungsvoll erwiesen hat: Kindgerechte Abläufe, kindgerechtes Ambiente, medikamentöse Analgesie. Eltern und Kinder müssen im Vorgespräch ausführlichste Informationen über die Abläufe erhalten (Rollenspiel, Modeling). Unangenehme Begleitfaktoren vor der Narkoseeinleitung sind zu vermeiden. Das tragende Element der medikamentösen Analgesie ist die Regionalanästhesie; hinzu kommen Paracetamol und NSAR (Höchstdosierungen beachten!) sowie Opioide. Bei Opioidanwendung dürfen zum einen keine Abstriche an den Überwachungsstandards im Aufwachraum inkl. Pulsoxymetrie gemacht werden, zum anderen ist wegen der bei perioperativer Opioidgabe deutlich erhöhten Rate an postoperativer Übelkeit und Erbrechen eher der untere Dosisbereich zu wählen. Perioperative Analgesiekonzepte werden im Detail vorgestellt.
