Schmerzpsychologische Interventionen bei onkologischen Patienten

Ein systematisches Review zum aktuellen Stand der Forschung

Zusammenfassung

Hintergrund

Schmerzen gehören zu den häufigsten Symptomen einer Krebserkrankung. Besonders im späten Erkrankungsstadium gehen diese Schmerzen mit beträchtlichem Leid einher. In der Psychoonkologie wurden erste schmerzpsychologische Ansätze erprobt. Allerdings fehlt ein systematischer Überblick über die Interventionen, besonders mit Blick auf deren klinische Anwendungen, Evidenzbasierung, methodische Qualität und den Vergleich mit etablierten Behandlungen. Ziel der Arbeit ist ein systematisches Review zum aktuellen Forschungsstand der psychologischen Schmerztherapie bei onkologischen Patienten.

Material und Methoden

Die Literatursuche erfolgte im Mai 2014 in den Datenbanken MEDLINE, PsycINFO, PSYNDEX und CENTRAL. Eingeschlossen wurden schmerzpsychologische Interventionen für erwachsene Krebspatienten, die in randomisierten, kontrollierten Studien (RCT) untersucht und bei denen Schmerzen als primärer oder sekundärer Endpunkt erhoben worden sind. Nach Prüfung auf Einschluss wurden Daten strukturiert extrahiert und bewertet. Die Datensynthese erfolgte narrativ.

Ergebnisse

In das Review wurden 32 RCT eingeschlossen. In den Studien wurden meist Patientinnen mit Mammakarzinom bzw. Patienten in frühen Erkrankungsstadien untersucht. Die methodische Qualität der eingeschlossenen Studien war heterogen. Meist wurden Kurzinterventionen im ambulanten Setting von Pflegekräften angeboten. Edukative und Entspannungsinterventionen, gefolgt von verhaltenstherapeutischen und kognitiven Elementen, waren die am häufigsten eingesetzten Komponenten.

Schlussfolgerungen

Forschungsbedarf besteht hinsichtlich der Wirksamkeit aktueller psychotherapeutischer Verfahren bzw. von Mediatorvariablen (z. B. Coping) für die Schmerzwahrnehmung bei Krebspatienten. Es fehlt an Studien mit hoher methodischer Qualität, in denen Intervention und Design umfassend und transparent berichtet werden.
